Student COVID-19 Self-Referral
This form is for students to notify Penn State Contact Tracing if they have tested positive for COVID-19 or believe they are a close contact of someone that has tested positive for COVID-19. If you are experiencing COVID-19 symptoms, please seek medical attention at University Health Services, your campus health center, or a primary care provider. This referral is not for medical advice or treatment.
General Contact Tracing Referral
This form is to refer students who have tested positive for COVID-19 or been exposed due to close contact with an individual that has tested positive to Penn State Contact Tracing. We will review the information provided and follow up with the referred student as appropriate.
