This is a list of items that must be completed prior to Fall 2026 start. Start requirements after March 1, 2026. Do not wait to start these requirements. Add the date completed before each task.

Shadowing, Information Sessions, and New Student Orientation

____________ Complete 8 hours shadowing and fill out shadowing observation form (See RadSci FAQs for instructions on scheduling shadowing)

____________ Purchase DISA Healthcare account (an online records management system)—select Greater Allegheny and the FT58 package.

____________ Attend a mandatory informational session

____________ Review Penn State NSO tasklist and complete the ALEKS placement testing if required. The tasklist will be sent after paid acceptance to your email that you put on your application. Please make sure to regularly check your email!

____________ Attend the mandatory RadSci NSO on June 22, 2026

 

Clearances, Background Checks, and Liability Insurance

____________ Follow instructions on DISA Healthcare to complete PA Criminal History Check-results will be automatically uploaded from DISA Healthcare

____________ Follow instructions on DISA Healthcare to complete FBI fingerprints Act 73 and then upload results to DISA Healthcare

____________ Follow instructions on DISA Healthcare to complete the PA Child Abuse Act 33/34 and then upload results to DISA Healthcare

____________ Purchase liability insurance (to be renewed annually). Coverage limits must be $1,000,000/$3,000,000). Make sure to put September 1 as start date and upload to DISA Healthcare Can use any provider but many students use Professional liability insurance for students | HPSO

 

Immunizations

____________ Complete all required immunizations and upload actual lab result documentation to DISA Healthcare which include the following:

____________ Two step TB skin test 1-3 weeks apart or quantiferon gold blood test lab report uploaded to DISA Healthcare. If either of these are positive need to submit results of a clear chest x-ray

____________ Documentation of DTAP vaccine within last ten years uploaded to DISA Healthcare

____________ Documentation of Varicella vaccine or Titer uploaded to DISA Healthcare

____________ Documentation of MMR vaccines or Titer uploaded to DISA Healthcare

____________ Documentation of Hepatitis C Titer uploaded to DISA Healthcare

____________ Documentation of Hepatitis B vaccine series and positive antibody Titer uploaded to DISA Healthcare

____________ Documentation of COVID-19 vaccination to include vaccine manufacturer uploaded to DISA Healthcare if the vaccine has been completed (optional but highly recommended)

____________ Annual Flu vaccine (to be completed between August 24-Sept 7, 2026)

 

Physical Form, Health History, and Medical Insurance

____________ Current Medical Insurance card (both sides of card) with your name on it uploaded to DISA Healthcare. If the card does not have your name on it, you must upload the card and also a letter from the insurance provider stating that you are covered under the plan.

____________ Complete the Penn State Online Health History Form by logging in to—myUHS | Penn State Student Affairs (psu.edu)

____________ Complete the Radiological Sciences Physical Form within 6 months of program start date -link to physical form rad sci physical form.pdf
Upload completed form to DISA Healthcare

 

Uniforms

____________ Purchase two sets of positional lead markers (left markers are blue, red markers are right) Aluminum Vertical Position Markers 1/2" L & R With Initials PAP03-V (universalmedicalinc.com)

____________ Purchase uniforms and lab jackets through PSGA Bookstore

____________ Purchase appropriate black shoes for clinical-Clinical shoes or athletic shoes with all black or black with any color emblem or soles (clogs and sandals are not acceptable. Full shoe crocks are permitted.)

 

These items will be completed after classes begin in the Fall of 2026

____________ During Program Orientation on campus-complete CPR for BLS Healthcare Workers (Basic Life Support for Health Care Workers-This will be coordinated through campus and will have a fee) Upload documentation to DISA Healthcare

____________ Purchase drug testing from Castlebranch (PG43dt) and complete drug testing. SEPTEMBER ONLY—DO NOT DO THIS UNTIL INSTRUCTED TO DO SO AFTER CLASSES START IN THE FALL. Upload documentation to DISA Healthcare

____________ Complete the My Clinical Exchange for AHN (this will be done on campus but students will have to pay the fee annually–approximately 40.00/year)

____________ Purchase Rad Tech Boot Camp (follow instructions on first day of class)

Make sure to keep all original copies as you may need to provide originals to the clinical sites. If all information is not properly completed and uploaded to DISA Healthcare, you may not be able to begin the program in the fall.
