To enroll in an independent study, you must first complete this form. It is required for obtaining approval for independent study or research classes (294, 296, 494, and 496) each semester. Ensure all sections of this form are completed and submitted to the Chief Academic Officer for approval.

Before filling out the form, please gather the following:

  1. Approval from your academic adviser
  2. Approval from the faculty member supervising your independent study
  3. A brief description of the independent study
  4. A syllabus for the independent study
  5. The course name and number
  6. The number of credits you are registering for
